Community based HIV voluntary counseling and testing-same day result

This study aimed to study the service mole of Community Based HIV Voluntary Counseling and Testing (CB-VCT) with same resulting; HIV infection rate; and the HIV related risk behaviors among the clients. The CB-VCT consisted of 3 main components which are Community Mobilization (CM), Mobile Voluntary Counseling and Testing (MVCT), and Post Test Support Services (PTSS). The CM focused on the outreach education team, volunteer network and the community leader involvement. The MVCT focused on anonymous, free, risk reduction counseling approach, rapid test blood testing with same day resulting and ethnic language translators. The PTSS focused on coping effectiveness training, information sharing group, stigma reduction skill building workshop, social activities counseling and refer.
A total of 18,835 clients received counseling, of which, 96.52% were volunteered to take blood testing and 98.96% came back to get testing results. The HIV infection rate was 1%. One hundred and fifty five (152) cases were referred and no serious negative impact was founded. The HIV related risk behavior which had statistically significant difference to the HIV infection rate were STI, drug use and IDU experiences. The reasons for receiving the services consisted of personal reasons and service approach. The personal reasons were divided into 6 different groups. The first group was those who assess risk from sexual relation. The second group was those who assess risk from their jobs. The third group was those who want to confirm negative. The forth group wears those who want to be a tested model. The fifth group was those who felt being suspected HIV positive by neighbors so they intended to show the others. The sixth group was those who made decision after getting more information. Regarding VCT service approach, the main attractive approaches were short distance, reduce time consuming, free of charge, less blood drawn, extra time services, same day resulting and no language barrier. Regarding stress, among the positive clients, 48.26% were worried on stigma, 12.21% were worried on negative relation to the others, 9.30% were worried on disclosure and 4.07% were worried on job firer. Among general clients, stress was found among those who assess themselves under risky condition. There were fear to be infected, fear to be asked the result, insecure in confidentiality, stress from being stigmatized, and the stress due to the remaining risk both spouses and jobs. For coping those stress, they chose to receive latest queue post-test counseling, ignored neighbor reaction, seeking more information and accepting unavoidable risk.
