Participation service of Health Promoting Hospital Subdistrict for patients with chronic diseases, hypertension and diabetes Nongmuangkai, Phrae Province

Objective: To assess the outcomes of participation service of Health Promoting Hospital Subdistrict forpatients with chronic diseases, hypertension and diabetes Nongmuangkai Phrae Province including clinical outcomes(blood pressure and blood sugar levels) and patient’s satisfaction. Method: A retrospective study of 289 hypertensive and diabetic patients who received services in Nongmuangkai Hospital between February 2009 to January 2010 and then has been refered to the nearest health facility(Health Promoting Hospital Subdistrict/ Health Center) between February to September 2010 to compare the ability to control blood pressure and blood sugar levels before and after the hospital services. Results: The findings suggested that hypertensive patients could keep blood pressure levels as the target at 87.3 percent, which was not different from the services at the Nongmuangkai Hospital(p = 0.870), diabetic patients could keep blood sugar levels as the target at 54.0 percent, which was significantly lower than the services at the hospital(p = 0.010) and patients were satisfied with the service model more than 80 percent. Conclusion: Participation service of Health Promoting Hospital Subdistrict for hypertensive and diabetic patients were affected levels of blood pressure and blood sugar. However, this study also need to learn continuously for a long time because there are other factors that may affect blood pressure and blood sugar levels and need to update the guidelines for Health Promoting Hospital Subdistrict in caring for these patients.
