Abstract

           Background: Septicemia is a serious consequence of severe infections. In Lamphoon Hospital, patients with septicemia experienced a much higher mortality rate than patients with other medical diagnosis. Objectives: to explain the patients’ characteristics and to explore for characteristics associated with mortality. Design: Descriptive study. Setting: Lamphoon Hospital. Methods: The medical records of patients diagnosed with  septicemia between October 1st 2003 and September 30th 2004 were retrieved to obtain the patients’ characteristics, clinical profiles and outcomes. Patients characteristics were explained by descriptive statistics. Expired and survived patients were statistically compared by chi-squared test and the mortality risk characteristics were analysed and explained by risk ratios and 95% confidence intervals. Results: There were 355 patients diagnosed as septicemia, with female to male ratio of 1:1.1. Among those, 90.7% were from the medical department. Aged 46 years or over and 78.0% had underlying disorder(s). Sepsis comprised 25.4% severe sepsis 22.3% and septic shock 52.3%. Hemoculture was done in 72.4%, among which 37.0% were positive. Urinary tract infection, pneumonia and gastrointeslinal tract infection were most common. One to five organs failure/dysfunction was observed in 74.4%. The overall mortality was 47.9%. Mortality was increased in patients with more severe septic status, multiple organs failure/dysfunction, male gender, age of more than 75 years old, underlying disorder(s), chronic renal failure, negative hemoculture, pneumonia, gastrointestinal tract infection, non-identifiable source of infection and soft tissues. Conclusion: Patients with sepsis syndrome in Lamphoon Hospital were similar to other sepsis patients in general, without any additional striking characteristics that may be used to predict the patients’ prognosis. Therefore, in the treatment of these patients. Early diagnosis followed by early adequate treatment is still essential. More effective supportive treatment in patients with multiple organs failure/dysfunction may improve the patients’ survival.
