Gastroschisis at Chiang Rai Regional Hospital: A 5-year Experience in Surgical Management

Abstract

           Background: Two surgical managements of Gastroschisis at Chiang Rai Regional Hospital were one-staged primary closure and two-staged closure, while prognostic factors had not been studied. Objective: To report experience in surgical management of Gastroschisis’s infants at Chiang Rai Regional Hospital and study factors which were related to mortality. Method: Retrospective study, records of Gastroschisis’s infants between January 2000 – December 2004 in 27 infants, devised to one-staged primary closure group with 14 infants and two-staged closure group with 13 infants: were reviewed. Result: Of the 27 Gastroschisis’s operative infants. 1 of 14 in one-staged primary closure group was died (7%) 6 of 13 in two-staged closure group were died (46%). Mean Gastational age in survival and dead infants were 35 and 33 weeks respectively. One infant with associated congenital anomalies was survived while the others (3) were died. Low gestational age and associated congenital anomalies were significantly related to mortality Conclusion: One-staged primary closure in selective cases with gastroschisis had good outcome. Poor prognosis in gastroschisis was significantly depend on low gestational age and associated congenital anomalies.
