A retrospective study on clinical presentation of Leptospirosis in Mae Taeng Hospital, Chiang Mai province

Abstract

        This study investigated retrospectively 18 consecutive patients with serologically confirmed leptospirosis admitted during January 2002 to September 2005 in Mae Taeng hospital, Chiang Mai province. Study objectives were to document demographic data, clinical manifestation, laboratory findings and treatment regimen. Of the 18 patients male was found infection higher than female (14 and 4 or 3.5:1). Patients age ranged between 5 to 53 years old with 66.7% (12/18) were 30-49 years old. In addition 72.2% (13/18) of patients were employed and the occupation at risk was farmer 66.7% (12/18). July and August indicated high prevalence of leptospirosis in this area. On admission, the most symptoms were fever (1 00%), which 33.3% higher than 39°C, acute headache (50%), severe headache (22.2%), muscle pain (55.6%), calf tenderness (22.2%), red eyes (22.2%), stiff neck (5.6%) and jaundice (22.2%). Pertinent laboratory finding included white blood count higher than 10,000 cell/mm 3 (50%), platelet lower than 100,000 /mm 3 (16.7%), urine albumin greater than +1 (50%) which 16.7% higher than the normal level of BUN, creatinine (16.7%) and Billirubin (16.7%). The average admitting peroid was 5.9 days, which the longest of 11 days. 16.7% were transferred to provincial hospital and the rest of patients were completely cured, 83.3%, there were no death reported in this study. The patients were treated with Penicillin G Sodium (83.3%), Chloramphenicol (38.9%), and Doxycycline (27.8%) respectively.
