This one-group pretest–posttest quasi-experimental study aimed to evaluate the effects of a health literacy program for fall prevention behaviors and physical fitness among older adults at the Taksin Family Care Clinic, Tak Province. Participants were purposively selected from individuals aged 60 years and above. 46 people were enrolled and 37 people completed both pre- and post-intervention assessments (retention rate 80.43%). Data were analyzed using descriptive statistics, Paired Sample t-test, and Exact McNemar Test. The sample was predominantly female (70.27%), with a mean age of 71.24 years (S.D. = 7.07); 78.37% had primary-level education or lower and 37.84% were at risk of falling. Following the 7-week intervention, three findings revealed: (1) overall health literacy scores, including-three domains-comprehension, appraisal, and application of health information (p<0.001); (2) overall health behavior scores did not differ significantly between pre- and post-intervention; and (3) physical fitness showed a favorable trend, most notably in the 3-Meter Walk Test. The pass rate increased from 70.27% to 86.49%, but no statistically significant differences were found in any measure. The program significantly enhanced health literacy. However, 7 weeks was insufficient to produce significant changes in health behaviors or physical fitness. Future programs should extend to at least 11-12 weeks and adopt a randomized controlled trial design to confirm effectiveness.
