Translation and Psychometric Validation of the Thai Version of the Kidney Symptom Questionnaire (KSQ) for Patients with Chronic Kidney Disease Stages 3-5

Chronic kidney disease (CKD) leads to the accumulation of metabolic waste products, resulting in a wide range of distressing symptoms that adversely affect patients’ well-being. Accurate symptom assessment is essential for effective clinical management and care planning. This study aimed to translate the Kidney Symptom Questionnaire (KSQ) into Thai and to evaluate its psychometric properties in terms of content validity and reliability. A descriptive methodological design was employed in two phases. Phase I involved forward and backward translation procedures conducted independently by professional translators from two university-based language institutes. Content validity was subsequently assessed by a panel of five experts. Phase II examined the reliability of the Thai version of the KSQ in a sample of 60 patients diagnosed with CKD stages 3–5, recruited from nephrology outpatient departments and CKD clinics at a tertiary care hospital in Bangkok, Thailand. Data were analyzed using Cronbach’s alpha coefficient. The findings demonstrated that the Thai version of the KSQ was semantically equivalent to the original English instrument. The content validity index was excellent (CVI = 1.00), and the internal consistency reliability was high (Cronbach’s alpha = 0.89). The most commonly reported symptoms were fatigue (90.00%), sleep disturbance/insomnia (88.33%) and itching (86.67%). In conclusion, the Thai-translated KSQ exhibits strong psychometric properties and is a reliable and valid instrument for assessing symptom experience among patients with CKD stages 3–5 in the Thai context.
