Safety and cost-benefit of self-made atraumatic fishing nylon for surgical wound closure, compared with commercial nylon Ethilon ®

This study aim to evaluate a safety and cost-benefit of self-made atraumatic suture, fishing-nylon, compared with those of commercial nylon. Eighty patients who had acute non-ruptured appendicitis were recruited with randomly received different type of material sutures for skin closure, self-made atraumatic fishing nylon and commercial nylon respectively. Both material sutures were sterilized with ethylene oxide. Clinical tissue reaction from each type of sutures were evaluated and compared 1n the 1 , 2 , 3 and 7 post-operat1ve days. Cost-benefit and surgeons satisfication were also evaluated. The results show that clinical tissue reaction of the self-made fishing nylon was not significantly different from those of commercial nylon, even though subgroup analysis was performed (p>O.OS).Self-made fishing nylon cost about 7-10 baht per pack which is less than ten folds of cost of the commercial nylon. All surgeons satisfy with the self made atraumatic fishing line because it is easy to use. We concluded that tissue reaction from fishing nylon was not different from those of commercial nylon. However the process of sterilization was very important, ethylene oxide and formaldehyde sterilization are recommended. Advantages of self-made atraumatic fishing nylon were low cost, locally producible, acceptable safety level and sharpness of injection needle. Self-made atraumatic fishing nylon is suggested to be an alternative of suture materials for skin closure for surgical wounds particularly in the hospitals where the similar
situation was found.
