การพัฒนาระบบช่องทางด่วนพิเศษ (ACS Easy Pass Model) ต่อผลลัพธ์ทางคลินิกและระยะเวลาการเปิดหลอดเลือดหัวใจในผู้ป่วยภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลัน

วัตถุประสงค์ : เพื่อศึกษาประสิทธิผลการให้บริการตรวจรักษาผู้ป่วยภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลันแบบช่องทางด่วนพิเศษ (ACS Easy Pass Model)
รูปแบบและวิธีวิจัย : เป็นการศึกษาย้อนหลังในผู้ป่วยภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลันที่มีความเสี่ยงสูงจำนวน 207 ราย ซึ่งเข้ารับการรักษาผ่านระบบช่องทางด่วนพิเศษที่ศูนย์หัวใจสิริกิติ์ภาคตะวันออกเฉียงเหนือ ระหว่างเดือน พฤษภาคม พ.ศ. 2561 ถึงเดือน กันยายน พ.ศ. 2566
ผลการศึกษา : ระบบบริการแบบช่องทางด่วนพิเศษ ผู้ป่วยจะได้รับการคัดกรองตามข้อบ่งชี้พร้อมแจ้งทีมห้องตรวจสวนหัวใจล่วงหน้าเพื่อเตรียมความพร้อมเมื่อถึงจุดนัดพบ และถูกส่งตัวเข้าห้องตรวจสวนหัวใจโดยตรงโดยไม่ต้องผ่านห้องตรวจฉุกเฉิน จากการศึกษาพบว่า ผู้ป่วยภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลันที่มีความเสี่ยงสูงหรืออยู่ในภาวะวิกฤตที่เข้ารับบริการส่วนใหญ่มีภาวะช็อกจากหัวใจ (ร้อยละ 32.4) และภาวะหัวใจหยุดเต้นที่ช่วยฟื้นคืนชีพได้ (ร้อยละ 28.5) ตามลำดับ ซึ่งส่วนใหญ่เป็นผู้ป่วยกล้ามเนื้อหัวใจขาดเลือดเฉียบพลันชนิดคลื่นไฟฟ้าหัวใจเอสทียก (ร้อยละ 92.8) ด้านผลลัพธ์ทางคลินิกพบว่า ผู้ป่วยได้รับการทำบอลลูนขยายหลอดเลือดหัวใจ ร้อยละ 87.9 พบภาวะช็อกจากหัวใจ ร้อยละ 27.1 ก้อนเลือดบริเวณจุดใส่สายสวน ร้อยละ 22.7 หัวใจเต้นผิดจังหวะ ร้อยละ 18.4 อัตราการเสียชีวิต ร้อยละ 16.9 และเวลามัธยฐานของวันนอนโรงพยาบาลอยู่ที่ 5 วัน (Q1, Q3 = 3, 9) เวลาเฉลี่ยที่เปิดหลอดเลือดหัวใจ 24 นาที (ต่ำสุด 5 นาที และสูงสุด 69 นาที)
สรุปผลการศึกษา : ระบบบริการแบบช่องทางด่วนพิเศษส่งผลดีต่อการเข้าถึงการรักษาที่รวดเร็ว ช่วยลดระยะเวลาการเปิดหลอดเลือดหัวใจได้อย่างมีประสิทธิภาพ ระบบนี้จึงมีความเหมาะสมในการนำไปประยุกต์ใช้เพื่อเพิ่มคุณภาพการดูแลผู้ป่วยภาวะกล้ามเนื้อหัวใจขาดเลือดเฉียบพลันในสถานการณ์วิกฤต
