Primary Cutaneous Plasmacytosis: Successful Treatment with Methotrexate

Abstract

Background: Primary cutaneous plasmacytosis (PCP) is a rare skin disorder which occurs mainly in Asian populations, especially Japanese adults. Polyclonal proliferation of plasma cells and hypergammaglobulinemia are its characteristic findings. The disease is limited to the skin with no involvement of lymph nodes or other organ systems. PCP is typically resistant to therapy without spontaneous remission. We present a 69-year-old Thai male presented with progressive asymptomatic infiltrative erythematous to brownish papules and nodules on upper trunk and back for over three years.Skin biopsy revealed dense infiltration of plasma cells and lymphocytes. Serum protein electrophoresis showed polyclonal hypergammaglobulinemia. The patient was diagnosed as PCP which was treated successfully with methotrexate.To the best of our knowledge, the treatment with methotrexate has never been reported.
