The Effect of Web Application Used for Developing a Medication Reconciliation System Between Pharmacy Units and Patient Referral Center within Hospital Network

Background: Guided by a policy prioritizing seamless healthcare services, an analysis of the patient referral process revealed occasional deficiencies in inpatient medication data. Such gaps can lead to treatment delays and compromise care efficacy at the hospital nearby patient residence. In case of outpatients, the requirement for manual referral documents alongside electronic system entries creates redundancy and risks of data incompletion. Furthermore, continuity of care is disrupted when medications previously prescribed are not available in the receiving hospital's formulary or there is no suitable alternatives. In response to these challenges, the hospital developed a process involving web application for medication reconciliation.
Objectives: To compare the accuracy and the completeness of medication data in patient referral documents between the traditional paper-based format and the web application-based system, as well as to evaluate user satisfaction and investigate medication-related issues and feedback post-implementation of new system.
Method: This action research involved a retrospective study of patient referral records obtained from the hospital's referral center reporting system. Medication-related report were collected and user satisfaction of the new system were also assessed.
Results: The use of web application in a medication reconciliation achieved a 100% accuracy rate, and this was higher than the conventional system of 75.14%. Regarding medication-related referral issues, a prevalence of 1.64% was identified, primarily attributed to the unavailability of medications within the hospital formulary (66.67%) of nearby-patient residence hospital. Furthermore, the overall assessment of user satisfaction with the application was rated at a high level.
Conclusions: Use of web application in the medication reconciliation of referral process promotes seamless continuity of care.
