การประเมินสุขภาพผู้สูงอายุอย่างครอบคลุมในระยะเปลี่ยนผ่าน จากโรงพยาบาลสู่บ้าน: สรุปแนวคิดและหลักฐานเชิงประจักษ์

การประเมินภาวะสุขภาพผู้สูงอายุอย่างครอบคลุมในระยะเปลี่ยนผ่านจากโรงพยาบาลสู่บ้านเป็นการประเมินผู้สูงอายุที่มีความเสี่ยง
ต่อภาวะแทรกซ้อนจากการนอนพักรักษาในโรงพยาบาลและการเข้ารับการรักษาซ้ำภายหลังจำหน่ายออกจากโรงพยาบาลการประเมินนี้ช่วยให้การวางแผนการดูแลและจัดการปัจจัยเสี่ยงของผู้สูงอายุได้อย่างทันท่วงทีโดยสหสาขาวิชาชีพ การทบทวนวรรณกรรมนี้มีวัตถุประสงค์เพื่อทบทวนหลักฐานเชิงประจักษ์เพื่อสรุปและเสนอแนวคิดเกี่ยวกับการประเมินสุขภาพผู้สูงอายุอย่างครอบคลุมสำหรับการดูแลผู้สูงอายุในระยะเปลี่ยนผ่านจากโรงพยาบาลสู่บ้าน โดยค้นหาวรรณกรรมจากฐานข้อมูล
5 ฐาน ได้แก่ CINAHL PubMed ScienceDirect และ Google Scholar โดยใช้คำสำคัญในการค้น คือ Comprehensive geriatric assessment Discharge plan Older adults และ Transitional care จากการสืบค้นพบบทความทั้งหมด 45 เรื่อง
ใช้วิเคราะห์ทั้งหมด 19 เรื่อง โดยได้คัดบทความที่ไม่ตรงตามวัตถุประสงค์หรือไม่สามารถเข้าถึงข้อมูลฉบับเต็มได้ออก จากนั้นผู้เขียนแต่ละคนจะทำการอ่านและประเมินระดับงานวิจัยแต่ละเรื่อง ผลการประเมินได้ระดับ 1 จำนวน 17 เรื่อง และอีก 2 เป็นงานในระดับ 2 และระดับ 4 ตามลำดับ จากนั้นผู้เขียนได้ทำการสกัดเนื้อหาและเรียบเรียงผลการทบทวนร่วมกัน พบว่าการประเมินภาวะสุขภาพ
ผู้สูงอายุอย่างครอบคลุมในระยะเปลี่ยนผ่านไม่มีรูปแบบที่ตายตัว หน่วยงานสามารถเลือกใช้เครื่องมือหรือพัฒนาแบบประเมินสุขภาพผู้สูงอายุอย่างครอบคลุมที่เหมาะสมกับบริบทของหน่วยงานตนเองได้ แต่แนะนำให้ประเมินให้ครอบคลุมทุกมิติ ได้แก่ ด้านปัญหาสุขภาพ สุขภาพกาย สุขภาพจิตใจและสติปัญญา ความสามารถในการทำหน้าที่ และด้านสิ่งแวดล้อม การประเมินควรมีการจัดตารางการประเมินอย่างเป็นระบบตั้งแต่แรกรับจนถึงวันจำหน่ายออกจากโรงพยาบาลครอบคลุมไปถึงการติดตามอาการเมื่อผู้สูงอายุอยู่ที่บ้าน โดยบุคลากรทางการแพทย์ที่ทำการเยี่ยมบ้าน หรือการติดตามผู้ป่วยโดยใช้เทคโนโลยีสุขภาพทางไกลที่อาจเปิดโอกาสให้ผู้ป่วย ผู้ดูแล และอาสาสมัครเข้ามามีบทบาทในการประเมินได้
