Abstract

Purpose: This study aims to develop and improve the patient counseling process for bowel preparation before colonoscopy for colorectal cancer screening by comparing the outcomes between inpatients and outpatients at Somdej Phra Yupparat Hospital, That Phanom.

Study design: This is a descriptive and analytical study using both quantitative and qualitative data from a comparison between inpatient and outpatient groups, along with focus group discussions with healthcare personnel.

Methods: The study was conducted at Somdej Phra Yupparat Hospital, That Phanom, by collecting data from 398 patients between 2022 and April 2025. Data were collected from medical records and focus group discussions with healthcare personnel and analyzed using descriptive statistics and Chi-square tests.

Main findings: The bowel preparation for colonoscopy in inpatients was found to be clean and adequate in 85.2%, while in outpatients, only 72.1% had adequate bowel preparation (p-value < 0.05). Outpatients had a higher re-examination rate of 7.2%. Focus group discussions revealed that non-compliance with bowel preparation instructions was a major issue, and suggested the use of checklists and follow-up calls to patients.

Conclusion and recommendations: Proper bowel preparation improves the effectiveness of colorectal cancer screening, reduces the need for re-examinations, and increases accuracy. Patients should be followed up using checklists and educational materials to ensure proper bowel preparation.
