การศึกษาโรค Necrotizing Fasciitis ในโรงพยาบาลสมเด็จพระพุทธเลิศหล้า

บทคัดย่อ

Objective : To analyze the necrotizing fasciitis in Somdejpraputhleratla Hospital in terms of incidence, clinical feature, location of infection, sign and symptom, microbiology, comorbidity, treatment and risk factors for mortality

Material and method : Retrospective study was performed in 63 patients who were diagnosed as necrotizing fasciitis during October 2007 to September 2008.

Results : 63 patients were reviewed, male 34 and female 29, mean of age was 60.57. The most common sites of infection were loot (58.7%) and leg (31.7%). The common signs and symptoms were swelling, erythema, pain and bleb. DM. was the most comorbid disease (63.5%). The most causative organism was Staphylococcus aureus. Early debridement was done within 24 hour after patients were resuscitated and improved their underlying conditions. Seven patients were dead (11.1%). Statistic analysis revealed risk fac-tors of mortality were delayed debridement more than 24 hour and having more than one comorbidity which were significant at p = 0.008 and 0.036 respectively.

Conclusion : Necrotizing fasciitis was common founded in old age. Site of infection were foot and leg. Prominent signs and symptoms were swelling, erythema, pain and bleb those rapidly progressed. Early debri- dement reduced mortality whereas presenting more than one comorbidity increased mortality.
