บทคัดย่อ

วัตถุประสงค์: เพื่อศึกษาผลลัพธ์ของการฟื้นฟูสมรรถภาพผู้ป่วยโรคสมองขาดเลือดในระยะเฉียบพลัน ได้แก่ ค่าสมรรถภาพที่เพิ่มขึ้น ประสิทธิภาพและประสิทธิผลของการฟื้นฟูสมรรถภาพ ในกลุ่มที่ได้รับยาฉีดละลายลิ่มเลือดเทียบกับกลุ่มที่ไม่ได้รับยาฉีดละลายลิ่มเลือด

วิธีการศึกษา: สืบค้นข้อมูลของผู้ป่วยโรคสมองขาดเลือดในระยะเฉียบพลันจากเวชระเบียน ได้แก่ ข้อมูลพื้นฐาน จำนวนวันนอนโรงพยาบาล สมรรถภาพก่อนและหลังได้รับการฟื้นฟู (ผลต่างระหว่างคะแนนบาร์เธลก่อนจำหน่ายเทียบกับแรกรับ) ประสิทธิภาพการฟื้นฟู (อัตราส่วนระหว่างสมรรถภาพที่เพิ่มขึ้นเทียบกับระยะวันนอนโรงพยาบาล) และประสิทธิผลของการฟื้นฟูสมรรถภาพ (ร้อยละของอัตราส่วนระหว่างสมรรถภาพที่เพิ่มขึ้นเทียบกับผลต่างของคะแนนบาร์เธลแรกรับและคะแนนสูงสุด)

ผลการศึกษา: ผู้ป่วยโรคสมองขาดเลือดในระยะเฉียบพลัน จำนวน 76 ราย แบ่งเป็นกลุ่มที่ได้รับยาฉีดละลายลิ่มเลือดจำนวน 29 ราย (ร้อยละ 38) อายุเฉลี่ย 62.7±12.3 ปี และกลุ่มที่ไม่ได้รับยาละลายลิ่มเลือดจำนวน 47 ราย (ร้อยละ 62) อายุเฉลี่ย 64.1±12.7 ปี โดยไม่พบความแตกต่างทางสถิติของข้อมูลพื้นฐาน ลักษณะทางประชากร คะแนนบาร์เธลแรกรับ (6.48±3.68 และ 6.55±3.24; p=.931) และระยะเวลาการนอนโรงพยาบาล (14.67±9.32 และ 13.47±7.96;  p = .672) ระหว่างทั้งสองกลุ่มภายหลังการฟื้นฟูสมรรถภาพพบว่าค่าเฉลี่ยสมรรถภาพเพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติ (p = .029) ในกลุ่มที่ได้รับยาฉีดละลายลิ่มเลือด (6.93±2.37) เมื่อเทียบกับกลุ่มที่ไม่ได้รับยาฉีดละลายลิ่มเลือด (5.57±2.76) และมีประสิทธิผลการฟื้นฟูสมรรถภาพที่ดีกว่า (53.80±19.07 และ 43.87±22.31; p = .043) แต่ไม่พบความแตกต่างอย่างมีนัยสำคัญทางสถิติประสิทธิภาพการฟื้นฟูสมรรถภาพ (0.69±0.52 และ 0.57±0.45; p = .183) 

สรุป: ผู้ป่วยโรคสมองขาดเลือดในระยะเฉียบพลัน ในกลุ่มที่ได้รับยาฉีดละลายลิ่มเลือดมีค่าเฉลี่ยสมรรถภาพที่เพิ่มขึ้นหลังการฟื้นฟูมากกว่าและประสิทธิผลการฟื้นฟูสมรรถภาพที่ดีกว่ากลุ่มที่ไม่ได้รับยาฉีดละลายลิ่มเลือด แต่ไม่พบความแตกต่างด้านประสิทธิภาพการฟื้นฟูอย่างมีนัยสำคัญทางสถิติ
