The Development and Learning Enhancement for Total Knee Arthroplasty Patients Under Nursing Care of Orthopedic Nurses at Nakhonpathom Hospital

Objective : To study the development and learning enhancement of total knee arthroplasty patients under nursing care at Nakhonpathom Hospital.
Method : The participatory action research was conducted by a group of 12 mentor nurses and 35 regular nurses who work together in the orthopedic ward from the period of February to June 2014. Sixty patients were classified into 2 groups; 30 before and 30 after developing the learning process.
Results : 1) During survey study, the orthopedic nurses found that the most effective learning methods were the group activities for knowledge exchange, knowledge identification and knowledge acquisition. 2) During process of participatory learning, the mentor nurses’ opinion was at a highest level in the mentor nurses’ roles and competency guide lines of total knee arthroplasty patients’ nursing care and experiential learning. 3) During the learning enhancement period, the mentor nurses believed that the most valuable contribution came from acting as a practitioner mentor and as resource of empowerment through resource knowledge. The regular nurses believed that the most valuable contribution from the mentor nurses were competent counseling, friendly support and effective on-the-job training. 4) After this development period, both the mentor nurses and the nurses improved their abilities drastically in total knee arthroplasty patients’ nursing care. The nurses’ ability increased significantly at a level of .05 and the level of blood loss severity decreased significantly also at the level of .05.
Conclusion : The development and learning enhancement for total knee arthroplasty patients’ nursing care of orthopedic nurses, using participatory learning composed with experience based learning and group activity, the mentor nurse role, empowerment, were the most effective in improving nursing care competency.
