Comparison of Conservative and Operative Treatment for Hypertensive lntracerebral Hemorrhage in Nakhonpathom Hospital

Abstract

The effectiveness of surgical treatment of hypertensive intracerebral hemorrhage remain controversy, especially in semicoma group (GCS 6-12). This study was purposed to compare the result between conservative treatment and surgical treatment in this group. 154 patients with hypertensive intracranial hemorrhage in Nakhonpathom Hospital between 2003 to 2007 were reviewed. Treatment was grouped into conservative and surgical. Glassgow Outcome Scales (GOS) was used to define the result into good (GOS 3-5), poor (GOS 2) and dead (GOS 1). We found that patients in the lowest status (GCS = 6) had poor result and high motality rate by both groups of treatment, whereas in the group of GCS=7-9 had better outcomes by surgical treatment. ln high GCS, the outcomes of patients were good and not significantly different regardless method of treatment.
