The Prevalence of Metabolic Abnormalities in Patients Receiving Anti-retroviral Drugs in Nakhonpathom Hospital

Abstract

This study determine the prevalence, clinical characteristics of metabolic abnormalities and their current drugs used in management in Thai patients treated for Human immunodeficiency virus infection, a retrospective study was performed on 15th  May 2006 -15th October 2006. Using simple random sampling method found 71 patients who received anti-retroviraI drugs at Nakhonpathom Hospital. Epidemilogical, clinical and immune status as well as Iabolatory data related to lipid and glucose metabolism were obtained from medical records. The metabolic syndrome was determined by using criteria adapted from Adult Treatment Panel (ATP) lll. Prevalence of metabolic syndrome was 5 case (7%). Clinical characteristics of metabolic syndrome were hypertriglyceridemia, low HDL cholesterol and hypertentions 3 case (60%) as well as hypertriglyceridemia, low HDL cholesterol and hyperglycemia 2 case (40%). Interestingly, this study found. that 47 case (66.2%) of the patients had at least 1 abnormality or metabolic alterations in either fat maldistribution, hypertension or lipid or glucose metabolism. In patients with metabolic alterations found dyslipidemia 43 case (78.2%). Especially hypertriglyceridemia had 36 case (54.5%) of dyslipidemia patients. The most frequently prescribed drugs for treatment of dyslipidemia, hypertension and hyperglycemia were Gemtibrozil, Hydrcchlorothiazide and metformin, respectively. Especially in this study found patient with metabolic syndrome did not receive treatment in 1 case and patient with metabolic alterations did not receive treatment in 25 case, In conclusion, this study revealed high prevalence of metabolic abnormalities in patients treated with anti-retroviral drugs, which could lead to increase risk in cardiovascular disease.
