Health Related Quality of Life in Stroke Survivors

Objectives: To study Health Related Quality of Life (HRQol) and identify factors that determined poor HRQoL in 3 -month stroke survivors.
Materials and methods: In a cross –sectional study, 201 in-patients were prospectively enrolled then assessed 3 months post stroke at rehabilitative clinic, Trat Hospital. After case identification, motor impairment, cognitive deficit, depression, and walking velocity were tested by using the Stroke Rehabilitation Assessment of Movement (STREAM), Thai Mental Status Examination (TMSE), Patient Health Questionnaire-9 (PHQ-9) Thai version and 10 Meter Walking Test. HRQol was assessed using the Stroke Impact Scale-16 (SIS-16) Multivariable logistic regression analysis was done to explore the factors which influenced poor HRQoL (SIS-16 <75).
Results: The mean age of the patients was 64.8±12.0 years, mainly male 57.7%, mild to moderate stroke 85.1%, cerebral infarction 83.1%. Comorbidities were hypertension 73.6%, diabetes mellitus 25.9%, and smoking 28.4%. At 3 months the mean score of SIS-16 was 61.2±31.4 , good HRQol (ie. independent living) was 42.3 %, presence of cognitive deficit 52.2% and depression 42.8% unable to walk 17.4%. Age, admission NIHSS-T and STREAM significantly determined poor outcome HRQoL (Adjusted Odd Ratio 1.087, 1.373, 0.871, p<0.05).
Conclusion: Age, severity of stroke and motor impairment were identified as independent determinants of poor HRQol of 3-month stroke survivors.
