Abstract

Objective: Prescribing errors are preventable events and represent a major threat to patient safety. This study aimed to identify factors associated with prescribing errors in the outpatient pharmacy setting at Cha-am Hospital.

Methods: A retrospective analytic study was conducted using outpatient prescriptions containing at least one medication retrieved from the electronic medical record system between 1 October 2023 and 30 September 2024. A total of 118,666 prescriptions were included. Factors associated with prescribing errors were analyzed using univariate analysis and multiple logistic regression. Results were reported as adjusted odds ratios (OR_Adj) with 95% confidence intervals (95% CI).

Results: A total of 655 prescribing errors were identified, corresponding to an incidence rate of 5.5 per 1,000 prescriptions. Factors significantly associated with prescribing errors in the multivariable analysis included prescriber’s years of experience (OR_Adj = 0.95); number of medications per prescription (OR_Adj = 1.12); timing of prescription (OR_Adj. = 2.15); certain patient age groups; history of drug allergy (OR_Adj = 1.52); absence of current medication history (OR_Adj = 7.55); and estimated glomerular filtration rate (eGFR) <60 mL/min/1.73 m², which was significantly associated with a lower risk of prescribing errors (OR_Adj = 0.56).

Conclusion: Prescribing errors were associated with multiple factors related to prescribers, patients, and health information systems. The findings suggest that incorporating identified risk factors into the development of computerized clinical decision support systems (CDSS) may play an important role in reducing prescribing errors and enhancing medication safety within healthcare systems.
