Prevalence of Polypharmacy and Polypharmacy-Related Factors Among Older Adult Medical Inpatients at Ratchaburi Hospital

Abstract

Objective: To examine the prevalence of polypharmacy, drug duplication, drug interactions, strong anticholinergic drug use, and polypharmacy-related factors in older adult medical inpatients at Ratchaburi Hospital

Methods: A cross-sectional study of older adult inpatients who admitted in the medical ward at Ratchaburi Hospital from July to September 2025.

Results: A total of 379 participants were included. The prevalence of polypharmacy was 64.9%, while the prevalences of drug duplication, drug interactions, and strong anticholinergic drug use were 4.2%, 33%, and 10% respectively. Polypharmacy-related factors were asthma/COPD (adjusted odds ratio 18.067), strong anticholinergic drug use (adjusted odds ratio 6.988), diabetes mellitus (adjusted odds ratio 5.298), ischemic heart disease (adjusted odds ratio 4.846), OPD visits ≥3 times/year (adjusted odds ratio 3.658), cerebrovascular disease (adjusted odds ratio 2.843), hypertension (adjusted odds ratio 2.586), number of physicians ≥3 /year (adjusted odds ratio 2.269), and BMI (adjusted odds ratio 1.076)

Conclusions: Polypharmacy is a common problem in older adults. Several factors associated with polypharmacy including patient-related factors, prescribing-related factors, and healthcare accessibility factors. Therefore, medication reconciliation and examination are important in order to lessen adverse health outcomes.
