Orthodontic Molar Uprighting to Avoid Prosthodontic Treatment: A Case Report

Uprighting the mandibular second molar combined with molar protraction to close extraction spaces presents a considerable orthodontic challenge, particularly in patients requiring removal of severely decayed mandibular first molars. This case report describes a successful orthodontic approach involving uprighting and mesial movement of the mandibular second molars to eliminate the extraction spaces, thereby reducing the need for prosthetic rehabilitation.The patient was a 12-year-old Thai female with severe dental crowding, maxillary incisor protrusion, and a mesially tipped, impacted mandibular second molars. Treatment was performed using a continuous straight-wire technique with superelastic nickel–titanium archwires, delivering light and continuous forces that enabled efficient molar uprighting. The results demonstrate that this simple and effective technique can be successfully applied to upright mandibular second molars and achieve space closure without prosthodontic treatment.
