Crown Lengthening Prior to Orthodontic Treatment in the Maxillary Anterior Region

Abstract

Short clinical crown is a condition that affects esthetics and can be an obstacle to dental treatment, particularly orthodontic bracket placement. Altered passive eruption is a significant etiological factor of short clinical crowns. Surgical crown lengthening plays a role in adjusting gingival-tooth ratio. Collaborative treatment planning between a periodontist and an orthodontist is therefore important in patients with this condition prior to orthodontic treatment to achieve optimal treatment outcomes.

This case report presents the management of short clinical crowns in the maxillary anterior region using surgical crown lengthening in a 25-year-old Thai female patient who sought treatment due to short crown lengths that prevented proper orthodontic bracket placement. Clinical examination revealed a central incisor length of 7 millimeters, accompanied by altered passive eruption. Following surgical crown lengthening, the clinical crown length increased to 10 millimeters. At the 3- and 12-month follow-ups, orthodontic treatment could be continued, and the patient was satisfied with the treatment outcome.
