Abstract

Objectives: To study factors associated with delayed hospital arrival time in acute stroke patients in Ratchaburi Hospital

Methods: This cross-sectional study collected the data from acute stroke patients included ischemic and hemorrhagic stroke at Ratchaburi Hospital. Medical record forms and data in HOSxP program were used as study tools. The data were presented in percentage, average, and standard deviation; t test independent, chi-square test, and logistic regression were implemented for the analysis of the factors associated with delayed hospital arrival time.

Results: Among 320 acute stroke patients, 193 patients had delayed hospital arrival time (60.3%). Factors associated with delayed hospital arrival time were symptom presented at home (adjusted OR = 3.71; 95% CI = 1.35–10.22; p = .011), wake-up stroke (adjusted OR = 7.47; 95% CI = 2.73–20.35; p < .001), and distance between the place that the symptom presented and the hospital (adjusted OR = 1.03; 95% CI = 1.00–1.06; p = .045). Factors associated with decreased time to hospital were knowledge about BEFAST of a relative (adjusted OR = 0.37; 95% CI = 0.15–0.89; p = .028), a patient who presented with facial palsy (adjusted OR = 0.43; 95% CI = 0.22–0.84; p = .014), a patient who presented with nausea and vomiting (adjusted OR = 0.46; 95% CI = 0.00–0.63; p = .021), and a patient who went to the hospital as soon as possible after symptom presentation (adjusted OR = 0.09; 95% CI = 0.01–0.18; p < .001).

Conclusions: Factors associated with delayed hospital arrival time in acute stroke patients were symptom present at home, wake-up stroke, and distance between the place that the symptom presented and the hospital.
