Efficacy of Tranexamic Acid in Addition to Active Management of the Third Stage of Labor for the Prevention of Postpartum Hemorrhage in Vaginal Delivery: A Randomized Controlled Trial

Abstract

Objective: To compare the efficacy of tranexamic acid (TXA) in addition to Active Management of Third Stage of Labor (AMTSL) versus AMTSL alone for the prevention of postpartum hemorrhage in vaginal delivery

Methods: A double-blind randomized controlled trial (RCT) was conducted at Phaholpolpayuhasena Hospital, Kanchanaburi Province. During May–September 2025. A total of 130 participants (65 per group) were enrolled. The control group received standard AMTSL; the treatment group received AMTSL plus 1 g intravenous TXA.

Results: Total blood loss within 24 hours was significantly lower in the control group (187.4 ± 76.7 mL) compared to the TXA group (213.4 ± 82.6 mL) (p = .017). Incidence of postpartum hemorrhage did not differ between groups (15.4% vs 12.3%; p = .800). No adverse events were observed in either group.

Conclusion: This study did not demonstrate clear evidence that prophylactic TXA administration in conjunction with AMTSL significantly reduced postpartum hemorrhage among low-risk parturients from a clinically significant perspective.
