The purpose of this retrospectivestudy was to describe the demographic pattern, signs and symptoms, criteria for operative treatment, complication and comparing cost between medical treatment and surgical treatment of benign prostatic hyperplasia. The 418 patients with benign prostatic hyperplasia at Nakhon Phanom Hospital between 2011 and 2012 were reviewed. The result showed that the most common age group were older than seventy years (42.8%), the most co-morbidity was hypertension with hypercholerterol (30.62%), the most common signs and symptoms were hesitancy, urgency and nocturia (64.6%) which required medical therapy. The more severe signs and symptoms were urinary retention, renal insufficiency, urinary tract infection, which required surgical therapy (26.3%). The complications of transurethral resection of the prostate (TURP) were bleeding (9%) and secondary infection (1.8%). No patient died from operations. In comparing head to head, surgical cost was lower than medical cost. Therefore, treatment with TURP surgery in patients with severe BPH seems to be the appropriate treatment for patients with BPH in Nakhon Phanom than medication in the long term
