ปัจจัยที่สัมพันธ์กับภาวะซึมเศร้าของผู้สูงอายุในพื้นที่เขตสุขภาพที่ 5

บทคัดย่อ

ภาวะซึมเศร้าในผู้สูงอายุเชื่อมโยงกับหลายสาเหตุแต่ไม่ชัดเจนว่าปัจจัยใดสัมพันธ์เชิงพยากรณ์กับภาวะซึมเศร้าของผู้สูงอายุ การวิจัยนี้เป็นการวิจัยแบบผสมผสานเชิงอธิบายแบบลำดับขั้น (explanatory sequential design) ซึ่งมีวัตถุประสงค์เพื่อศึกษาระดับภาวะซึมเศร้าของผู้สูงอายุ ปัจจัยสัมพันธ์เชิงพยากรณ์กับภาวะซึมเศร้าและลักษณะเชิงคุณภาพของปัจจัยพยากรณ์  กลุ่มตัวอย่าง คือผู้สูงอายุในเขตสุขภาพที่ 5 จำนวนทั้งสิ้น 480 คน  โดยได้จากการเลือกแบบเจาะจง กลุ่มตัวอย่างในวิจัยเชิงคุณภาพเลือกแบบเจาะจง 30 คนจากผู้มีภาวะซึมเศร้า  เครื่องมือวิจัยได้แก่ แบบสอบถามข้อมูลส่วนบุคคล แบบสอบถามสัมพันธภาพในครอบครัว แบบสอบถามการร่วมกิจกรรมทางสังคม แบบประเมินการนอนไม่หลับ แบบประเมินภาวะซึมเศร้าในผู้สูงอายุไทย และแบบวัดความเครียดสวนปรุง เครื่องมือวิจัย มีค่าความเชื่อมั่น สัมประสิทธิ์อัลฟ่าของครอนบาคเท่ากับ .80, .82, .85 และ .80 และ KR-21ได้ .83 ตามลำดับ การเก็บรวบรวมข้อมูลดำเนินการระหว่างวันที่ 1 พฤศจิกายน พ.ศ. 2567วันที่ 6 เมษายน พ.ศ. 2568 วิเคราะห์ข้อมูลเชิงปริมาณโดยหาความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และถดถอยพหุคูณ สำหรับข้อมูลเชิงคุณภาพวิเคราะห์โดยการหาประเด็นหลัก ผลการวิเคราะห์ข้อมูลเชิงปริมาณพบว่าคะแนนเฉลี่ยภาวะซึมเศร้าเท่ากับ 5.07 (SD=1.57) ซึ่งแปลผลว่าไม่มีภาวะซึมเศร้า สำหรับปัจจัยพยากรณ์ภาวะซึมเศร้าในภาพรวมสามารถทำนายได้ร้อยละ 68 (Adj R2=.68, p<.001) โดยประกอบด้วย การมีส่วนร่วมทางสังคม ความสัมพันธ์ในครอบครัว การนอนไม่หลับ และความเครียด (β =–.45,-.20, .15 และ .11 ตามลำดับ) ลักษณะเชิงคุณภาพของปัจจัยพยากรณ์ คือ ความขัดแย้งในครอบครัว รู้สึกโดดเดี่ยว หดหู่ใจที่นอนไม่หลับ และเครียดกับปัญหาชีวิต  ผลการศึกษาครั้งนี้บ่งชี้ถึงปัจจัยที่มีอิทธิพลต่อภาวะซึมเศร้าในผู้สูงอายุ ซึ่งบุคลากรที่เกี่ยวข้องควรคำนึงถึงเมื่อดำเนินงานที่มีเป้าหมายเพื่อลดความซึมเศร้าในผู้สูงอายุ
