การศึกษาระบาดวิทยาและผลการรักษาในผู้ป่วยที่มีภาวะเยื่อบุช่องท้องอักเสบติดเชื้อจากการล้างไตทางหน้าท้อง

บทคัดย่อ

ภาวะเยื่อบุช่องท้องอักเสบติดเชื้อจากการล้างไตทางหน้าท้องเป็นสาเหตุสำคัญของการเสียชีวิต โดยพบอุบัติการณ์ตั้งแต่ 0.24 ถึง 1.66 episodes per patient-year

วัตถุประสงค์การวิจัย : เพื่อศึกษาระบาดวิทยา ชนิดของเชื้อ ลักษณะทางคลินิกของภาวะเยื่อบุช่องท้องอักเสบติดเชื้อจากการล้างไตทางหน้าท้อง รวมถึงปัจจัยเสี่ยงที่ทำให้การรักษาล้มเหลว ในหน่วยล้างไตทางหน้าท้องทั้ง 5 หน่วยในจังหวัดกาฬสินธุ์

ระเบียบวิธีวิจัย : เป็นการศึกษาเชิงพรรณนาแบบตัดขวาง โดยทบทวนเวชระเบียนจากหน่วยล้างไตทางหน้าท้อง 5 โรงพยาบาลคือโรงพยาบาลกาฬสินธุ์ สมเด็จพระยุพราชกุฉินารายณ์ ยางตลาด กมลาไสยและสมเด็จ ในผู้ป่วยที่มีภาวะเยื่อบุช่องท้องอักเสบติดเชื้อจากการล้างไตทางหน้าท้อง ที่ได้รับการวินิจฉัยระหว่างวันที่1 ตุลาคม 2559 ถึงวันที่30 กันยายน 2560ผ่านการรับรองจริยธรรมการวิจัยในมนุษย์โรงพยาบาลกาฬสินธุ์เลขที่014/2017Eวิเคราะห์ด้วยสถิติเชิงพรรณนาและสถิติเชิงอนุมานด้วยโปรแกรมสำเร็จรูป

ผลการวิจัย : พบว่าอุบัติการณ์ผู้ป่วยที่มีภาวะเยื่อบุช่องท้องอักเสบติดเชื้อจากการล้างไตทางหน้าท้องคิดเป็น0.35episode per patient-year อายุเฉลี่ย 56 ปี (22-83 ปี) ระยะเวลาการล้างไตทางหน้าท้องตั้งแต่แรกจนเกิดติดเชื้อ คือ 647.5 วัน (12-2,918 วัน) โดย128 เหตุการณ์ของการตรวจเพาะเชื้อพบว่า ไม่พบเชื้อ 70 (54.6%), พบเชื้อแบคทีเรียแกรมบวก 29 (22.6%), เชื้อแบคทีเรียแกรมลบ 27 (21.0%) เชื้อแบคทีเรียแกรมบวกที่พบบ่อยคือStaphylococcus epidermidis, Staphylococcus aureusและ Streptococcus group D-not enterococciโดยพบ 9 (16.0%), 5 (8.9%) และ 5 (8.9%) ตามลำดับ ส่วนเชื้อแบคทีเรียแกรมลบที่พบบ่อย ได้แก่ Escherichia coli, Klebsiellapneumoniaeโดยพบ 13 (23.2%), 8 (14.2%)ตามลำดับ ผลการรักษาพบว่าการรักษาล้มเหลว 42 เหตุการณ์ (32.3%)จากการวิเคราะห์แบบ multivariable analysis พบ 2 ปัจจัยที่มีความเสี่ยงเพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติต่อผลการรักษาล้มเหลว ได้แก่ จำนวนเม็ดเลือดขาวจากการตรวจน้ำยาล้างไตทางหน้าท้องในวันที่ 3 ที่ระดับตั้งแต่ 1,090 เซลล์ต่อลบ.มม. (Odd ratio 6.28; 95%CI 2.43-16.21, p<0.001)และปริมาณปัสสาวะของผู้ป่วยที่น้อยกว่า 200 ซีซีต่อวัน (Odd ratio 3.51; 95%CI 1.46-8.42, p=0.005)
