ค่าความไวและความจำเพาะของผลเซลล์วิทยาจากการเจาะดูดด้วยเข็มขนาดเล็กเพื่อวินิจฉัยมะเร็งก้อนที่ต่อมไทรอยด์ โรงพยาบาลขอนแก่น

วัตถุประสงค์ : เพื่อศึกษาค่า sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV) และ diagnostic accuracy ของ FNA ใน thyroid เมื่อเทียบกับ Histology
รูปแบบและวิธีการวิจัย : รูปแบบการวิจัยเป็นการศึกษาวิจัยเชิงวิเคราะห์ (Analytic study by retrospective data collection) เก็บข้อมูลจากเวชระเบียนผู้ป่วยที่วินิจฉัย Thyroid nodule ที่ได้รับการทำ FNA และได้รับการผ่าตัด thyroid ในโรงพยาบาลขอนแก่น ระหว่าง 1 มกราคม 2562 - 31 กรกฎาคม 2564 จำนวน 123 ราย
ผลการศึกษา : ผู้ป่วยส่วนใหญ่เป็นผู้หญิง 88.6% อายุเฉลี่ย 48.7 ปี พบค่าความไว 36.36% ค่าความจำเพาะ 89.11% Positive likelihood ratio 3.34 Negative likelihood ratio 0.71 Positive predictive value 42.11% Negative predictive value 86.54% Accuracy 79.67%
สรุปผลการศึกษา : ค่าความไวและ positive predictive value ในการศึกษานี้น้อยเนื่องจาก false negative และ false positive ในการศึกษานี้มาก โดยผู้ป่วยรายที่เป็น false negative เนื่องจากเป็น papillary microcarcinoma และ cystic papillary carcinoma ส่วนในรายที่เป็น false positive เนื่องจากการพบ nuclear feature characteristic ของ papillary carcinoma ใน benign thyroid lesion
