Nursing Care of Unstable Brust fracture T12 with Surgical spine in elderly patient : Case Study

Abstract

Background : Thoracolumbar spine injuries  according to nervous system were found 15 -20 %. Spinal nerve injuries caused to paralysis according to injury morphology and neurological deficit. It has induced complications especially elderly patient with comorbidity disease, increased length of stay and high cost.

Objectives : To study nursing care of unstable Brun fracture T12

Methods : Selected a case study Unstable Brust fracture T12 with Surgical spine in elderly patient, collected data from document, interviewed the patient. Analysis data and health pattern problems, nursing care included admitted period, continuum care period and discharge period.

Result : The elderly patient with comorbidity disease ; Diabetes mellitus, Hypertension and Ischemic heart disease. Chief compliant had fallen from motocycle.  Diagnosis ; Unstable Brust fracture T12 with Decompression laminectomy T 11 - L1 with Pedicular screw fixation and Posterior lateral fusion T10 - L2. Nursing diagnosis has risk for Spinal shock, risk for  Spinal cord damaged, Back pain, Health pattern changed, Anxiety about illness and operation, Ineffective breathing pattern, Pain post operation, Risk for hypoglycemia and hyperglycemia, Anemia due to post operative blood loss, Fever, Constipation, Risk for infection of surgical wound and urinary tract infection, Risk for pressure sore and fall, Deficit of knowledge when discharge.

Outcomes : This patient could accepted and understanding her functional  health patterns and illness. She has no complication and needed continuous care and rehabilitation.
