A Spigelian hernia-cryptorchidism Syndrome : Case Report

Abstract

Spigelian hernia (SH) is a ventral interstitial hernia through a defect in the Spigelian fascia which rarely seen in pediatric age group. SH with undescended testis is considered to be a congenital condition and extremely rare. This association may be a distinct clinical syndrome of the defect in the Spigelian fascia, ipsilateral undescended testis, absence of gubernaculum and inguinal canal. The absence of the inguinal canal can make surgical complicated to place the undescended testis into the scrotum. A 2 - month-old boy presented with left Spigelian hernia and bilateral undescended testes. The SH sac was closed and the large defect was repaired with polypropylene mesh. The left undescended testis which was present in Spigelian hernia sac was brought to scrotum via the neo-tunnel medial to inferior epigastric vessels and just lateral to pubic tubercle. The patient was discharged in the next day with uneventful  and the right undescended testis which was palpated at inguinal area without any abdominal hernia can be waited until the appropriate time to do orchiopexy.
