Case report : multi-stage operation for giant inguinoscrotal hernia repair

Giant inguinoscrotal hernia was a rare condition which challenge for treatment due to problem of “loss of domain”. The internal organs were herniated into the scrotal sac. No standard treatment for this condition. We report a patient who presented with left giant inguinoscrotal hernia for 10 years. He had underlying disease of COPD. The operative finding was found entire small bowel, transverse, descending, sigmoid colon in scrotal sac. After adhesiolysis, the internal organs were transfer back from scrotal sac to abdominal cavity and left on abdominal wall with temporary abdominal closure technique. The following operations were slightly stretching, lengthening abdominal wall until closed abdominal wall successfully. The cardiopulmonary system of patient was gradually adaptation for carrying internal organs into his abdomen. No organ resection was performed. No need for special technique or equipment, so this procedure can be used in any hospital. The result was good and no complication.
