Unitcostofhealthcare service at QueenSirikit Heart CenteroftheNortheast, fiscalyear 2019

Abstract

Objective:This study aimed to analyzing the unit cost of the outpatient and inpatient services, and to comparing the cost per unit forthehealthcare services with the reimbursement ratein accordance with the Diagnosis Related Groups (DRGs).

Methods : A retrospective descriptive study was conducted.This study used the costs of outpatient and inpatient medical care services data of Queen Sirikit Heart Center in 2019 fiscal year from the perspective of the service provider. 

Result :The total direct costfor the Queen Sirikit Heart Centerin the fiscal year 2019 was 1,418,610,675.61 THB. The average unit cost of outpatient service for all service was 3,873.62 THB per visit.For the inpatient services, the average unit cost was 17,089.82 THB per day (Adj.RW), whichexcluded thecostsforprostheses and medical supplies. TheQueen Sirikit Heart Center hadreceivedrecompense for outpatient service, which waslower than the actual amount. The patient had used the KhonKaen Universal Health Care Coverage (UC), had been 37.43%. Similarly, the inpatient service, who had used the Comptroller General’s Department and the Department of Local Administration (DLA), UC outside and inside Regional Health Office 7, had been 62.19%, 56.17% and 33.91% respectively. 

Conclusion : The costunit of Queen Sirikit Heart Center was different from general hospital centers. The Queen Sirikit Heart Center received a reimbursement ratethat was insufficient for the actual cost per unit of service. Therefore, it should be used to plan future budgetfor financial stability.
