Mortality rate and risk factors in patients with blunt abdominal trauma: A retrospective study of Mukdahan Hospital

Objective : To investigate the mortality rate and identify the risk factors affecting the death of patients who experienced blunt abdominal trauma at Mukdahan Hospital.
Methods : This study is retrospective and descriptive. The population under investigation consisted of patients with blunt abdominal trauma who were admitted to the emergency room, survived, and received further treatment at Mukdahan Hospital from January 1, 2020, to September 30, 2023. The sample size was determined using the formula for estimating the proportion of a single population, resulting in 104 patients. Data were collected from medical records and analyzed using descriptive statistics. Inferential statistics were employed for analysis, including the Chi-square test, Independent t-test, Mann-Whitney U test, and logistic regression statistics.
Results : A total of 104 patients experienced blunt abdominal trauma, with a mortality rate of 5.8% (6 cases) and an average age of less than 34 years. The majority of accidents occurred while driving motorcycles. The most common joint injury system was appendage fracture, and the most frequently injured abdominal organ was the liver. When analyzing each variable, the significant factors affecting the death of patients with blunt abdominal trauma at the 0.05 level were intubation since the first admission (OR=8.80, 95% CI=1.56-49.58, p=0.014), consciousness score (GCS score) (OR=0.69, 95% CI=0.54-0.88, p=0.003), complications (OR=37.20, 95% CI=5.45-254.02, p<0.001), and serum bicarbonate (-HCO3) (OR=0.70, 95% CI=). However, during multivariate analysis using Multiple logistic regression, it was found that no variables had a statistically significant effect on the death of patients with non-penetrating intraabdominal injuries at the 0.05 level.
Conclusion : The mortality rate for patients with blunt abdominal trauma was 5.8%. When analyzing multivariates, factors affecting the death of patients with blunt abdominal trauma could not be identified. Further studies should be conducted with a larger population.
