Development of an Inpatient Drug Dispensing System at Mahasarakham Hospital

Abstract

Objectives :To develop an inpatient drug dispensing system and assess its impact on medication waiting times and dispensing error rates.

Methods :This action research was conducted between October 1, 2023, and March 31, 2024, comprising three phases: Phase 1, situational analysis and action planning; Phase 2, implementation of the newly developed dispensing workflow; and Phase 3, monitoring and evaluation of outcomes. The sample consisted of 31 hospital personnel involved in inpatient dispensing services (12 pharmacists, 9 pharmacy technicians, and 10 dispensary staff).  Data were collected from inpatient prescriptions before and after system implementation and analyzed using descriptive statistics. The Mann-Whitney U Test was employed to compare outcomes pre- and post-implementation

Results : Interventions included 1) streamlining the order transmission and internal workflow, and 2) updating the medication dispensing software. The results demonstrated a significant improvement: the waiting time for STAT medications dropped from 17.1+11.1 to 10.8+6.7 minutes (p < 0.01), and the waiting time for discharge medications during peak hours (10:30 AM – 1:00 PM) decreased from 40.7+17.6 to 24.5+13.8minutes (p = 0.02). Additionally, the dispensing error rate declined from 1.43 to 0.55 incidents per 1,000 patient-days per month.

Conclusion : The implementation of the developed inpatient drug dispensing system successfully reduced medication waiting times and dispensing errors, enhancing overall service efficiency.
