The Effects of Nursing Care Program Integrating SKT Meditation on Reducing Pain and Anxiety in Patients with Upper Gastrointestinal Bleeding in the Surgical Intensive Care Unit

Abstract

Objective : This study aimed to examine the effects of a meditation program (SKT) on reducing pain and anxiety among patients with upper gastrointestinal bleeding (UGIB).

Methods : A quasi-experimental research with a two-group pretest–posttest design was conducted. The participants were patients diagnosed with UGIB admitted to the Surgical Intensive Care Unit at Maha Sarakham Hospital between July 2024 and March 2025. A total of 40 participants were selected using purposive sampling and divided into an experimental group receiving the meditation program and a control group receiving routine nursing care. Research instruments included the SKT meditation program, a pain assessment scale, and the Spielberger State Anxiety Inventory. Data were analyzed using descriptive statistics and inferential statistics including Wilcoxon signed-rank test and Mann–Whitney U test.

Results : The experimental group demonstrated a significantly lower median pain score after the intervention (IQR = 0.00) compared with before the intervention (IQR = 1.00) (z = −3.99, p<0.050). The median anxiety score after the intervention (IQR = 3.75) was also significantly lower than before the intervention (IQR = 6.75) (z = −3.92, p<0.050). When compared with the control group, the experimental group had significantly lower median pain scores (IQR = 0.00) than the control group (IQR = 1.75) (z = −3.22, p<0.050). Similarly, the median anxiety score in the experimental group (IQR = 3.75) was significantly lower than that in the control group (IQR = 7.75) (z = −5.42, p<0.050).

Conclusion : The meditation program was effective in reducing pain and anxiety among patients with upper gastrointestinal bleeding. The program can be applied as a non-pharmacological nursing intervention to complement standard care in intensive care units by promoting relaxation, diverting attention from pain, and enhancing emotional balance, thereby reducing anxiety and improving patients’ psychological well-being.
