Development of a Follow-up System for Patients with Abnormal Cervical Cancer Screening Results in the Obstetrics and Gynecology Clinic at Mahasarakham Hospital

Objective : To develop and evaluate a proactive follow-up system for patients with abnormal cervical cancer screening results at the Gynecological Clinic, Mahasarakham Hospital.
Methods : This action research project followed the PAOR (Plan-Act-Observe-Reflect) cycle. The study participants comprised 30 patients with abnormal cervical cancer screening results who received services at the Gynecological Clinic, Mahasarakham Hospital, between January and December 2025. Research instruments included a demographic questionnaire, a cervical cancer knowledge test, a patient follow-up record form, a staff satisfaction questionnaire, and a semi-structured interview guide. Content validity was verified by experts (IOC range: 0.75–1.00), and the reliability of the knowledge test, calculated using the Kuder-Richardson Formula 20 (KR-20), was 0.85. Data were analyzed using descriptive statistics—frequencies, percentages, means, and standard deviations—and content analysis.
Results : The developed proactive follow-up system consisted of five key components: (1) establishing an individual patient registry system, (2) implementing a risk-stratified appointment system, (3) proactive follow-up led by nurse navigators, (4) multi-channel communication (telephone, Line Official Account, and SMS), and (5) integrating patient health education. Following the implementation of the system, the patient missed-appointment rate decreased significantly from 55.7% in 2024 to 11.5%, with a follow-up attendance/response rate of 76.7%. The majority of patients demonstrated a high level of knowledge (60.0%), with an overall mean knowledge score of 74.0% of the total score. Furthermore, healthcare personnel expressed a high level of satisfaction with the developed system ( = 4.2)
Conclusion : The action research based on the PAOR framework successfully developed a practical, proactive follow-up system in a gynecological clinic. The system significantly reduced the missed-appointment rate and promoted continuous care engagement. Long-term follow-up studies and scaling up the system to primary care networks are recommended to ensure sustainability.
