Mycobacterium marinum skin infection can be found in patients who exposed to contaminated water or soil. The lesions usually confined to the skin but can progress to other organs such as bone and joints in immunocompromised patients. We reported a 31-year-old healthy male presenting with multiple erythematous papules, nodules and plaques on abdomen arms and left thigh for 10 years. He had used methamphetamine in the past ten years. Several reports showed that methamphetamine can impair human immunity. We reported a case of extensive cutaneous Mycobacterium marinum infection with concurrent methamphetamine used.
