ประสิทธิผลของนวัตกรรมการป้องกันข้อติดเพื่อส่งเสริมพิสัยการเคลื่อนไหว และการดำเนินกิจวัตรประจำวันในผู้ป่วยหลังผ่าตัดข้อไหล่และข้อศอก: การวิจัยกึ่งทดลอง

บทนำ ภาวะข้อติดแข็งเป็นภาวะแทรกซ้อนที่พบได้บ่อยหลังการผ่าตัดข้อไหล่และข้อศอก ส่งผลกระทบต่อการดำเนินกิจวัตรประจำวันและคุณภาพชีวิตของผู้ป่วย แนวทางการฟื้นฟูสมรรถภาพปัจจุบันยังมีข้อจำกัดดังนี้ 1) การทำกายภาพบำบัดในโรงพยาบาลส่วนใหญ่ดำเนินการเพียงวันละ 1–2 ครั้ง เนื่องจากข้อจำกัดด้านจำนวนนักกายภาพบำบัด 2) เครื่อง continuous passive motion มีราคาค่อนข้างสูง การใช้งานซับซ้อน และต้องการพื้นที่จัดเก็บมาก จึงไม่เหมาะสำหรับการใช้งานในโรงพยาบาลขนาดเล็กหรือการนำกลับไปใช้ที่บ้าน และ 3) การทำ passive exercise แม้จะช่วยเพิ่มองศาการเคลื่อนไหวของข้อได้ แต่ไม่สามารถป้องกันภาวะกล้ามเนื้อฝ่อลีบได้เมื่อเทียบกับ active exercise และผู้ป่วยมักขาดความมั่นใจในการเคลื่อนไหวด้วยตนเอง แม้ว่าหลักฐานเชิงประจักษ์สนับสนุนการเคลื่อนไหวในระยะเริ่มต้นอย่างต่อเนื่องและมีความถี่สูง แต่ยังขาดเครื่องมือและนวัตกรรมที่ผู้ป่วยสามารถใช้เองได้อย่างปลอดภัยโดยไม่ต้องพึ่งพานักกายภาพบำบัดตลอดเวลา มีต้นทุนต่ำและสามารถผลิตได้ในท้องถิ่น มีโครงสร้างที่ชัดเจนช่วยให้ผู้ป่วยทำได้ถูกต้องและครอบคลุมทุกทิศทางการเคลื่อนไหว และสามารถใช้ได้บ่อย 6–8 ครั้งต่อวัน จึงจำเป็นต้องส่งเสริมการฟื้นฟูสมรรถภาพของผู้ป่วยหลังผ่าตัดข้อไหล่และข้อศอกให้มีประสิทธิภาพยิ่งขึ้น
วัตถุประสงค์การวิจัย การศึกษาครั้งนี้มีวัตถุประสงค์เพื่อศึกษาผลของนวัตกรรมการป้องกันข้อติดต่อพิสัยการเคลื่อนไหวของข้อและความสามารถในการดำเนินกิจวัตรประจำวันของผู้ป่วยหลังผ่าตัดข้อไหล่และข้อศอก
การออกแบบการวิจัย การศึกษาครั้งนี้เป็นแบบกึ่งทดลอง โดยประยุกต์แนวคิดทฤษฎีการเคลื่อนไหวและการฟื้นฟูสมรรถภาพ ที่มุ่งเน้นการเคลื่อนไหวข้ออย่างสม่ำเสมอต่อเนื่อง และส่งผลดังนี้ (1) รักษาและเพิ่มพิสัยการเคลื่อนไหว (2) ป้องกันการเกาะติดของเนื้อเยื่อ (3) ส่งเสริมการไหลเวียนเลือดบริเวณข้อ และ (4) กระตุ้นการทำงานของกล้ามเนื้อและเอ็น ร่วมกับแนวคิด Enhanced Recovery After Surgery (ERAS) ที่เน้นการเคลื่อนไหวในระยะแรก (early mobilization) และการมีส่วนร่วมของผู้ป่วย (active participation) นวัตกรรมการป้องกันข้อติดที่พัฒนาขึ้นในการศึกษานี้ออกแบบให้ผู้ป่วยสามารถออกกำลังกายแบบ active exercise ด้วยตนเองได้อย่างปลอดภัย โดยมีโครงสร้างที่ชัดเจน ประกอบด้วย 5 รูปแบบการเคลื่อนไหว ครอบคลุมทุกทิศทางของข้อไหล่และข้อศอก ได้แก่ การงอและเหยียดข้อ การยกแขนออกข้าง การหุบแขน และการหมุนข้อ โดยแบ่งเป็น 5 รูปแบบ คือ รูปแบบเส้นตรง รูปแบบระดับต่ำ รูปแบบระดับสูง รูปแบบโค้งเข้าหาตัว และรูปแบบโค้งออกจากตัว ผู้ป่วยปฏิบัติวันละ 6 ชุด เป็นเวลา 3 วันติดต่อกัน แต่ละชุดใช้เวลา 15–20 นาที โดยทำทั้ง 5 รูปแบบ รูปแบบละ 5 ครั้งต่อข้าง มีช่วงพัก 1–2 นาทีระหว่างรูปแบบ
วิธีดำเนินการวิจัย กลุ่มตัวอย่าง เป็นผู้ป่วยหลังผ่าตัดข้อไหล่และข้อศอกที่เข้ารับการรักษาในหอผู้ป่วยศัลยกรรมกระดูก โรงพยาบาลอุตรดิตถ์ จำนวน 60 คน เลือกแบบเจาะจงตามเกณฑ์การคัดเข้าและเกณฑ์การคัดออก ในช่วงระยะเวลาที่ทำการวิจัย คำนวณขนาดโดยใช้สูตรเปรียบเทียบค่าเฉลี่ยของสองกลุ่มอิสระ กำหนดค่าขนาดอิทธิพล (effect size, d) เท่ากับ 0.8 ตามการศึกษาที่ผ่านมาที่พบว่าการออกกำลังกายแบบมีโครงสร้างให้ขนาดอิทธิพลในช่วง 0.75–0.85 กำหนดค่าอำนาจการทดสอบ (Power) .80 และระดับนัยสำคัญ (α) .05 ได้ขนาดตัวอย่างกลุ่มละ 26 คน เพิ่มร้อยละ 15 เพื่อชดเชยผู้ที่ออกจากการวิจัย จึงได้กลุ่มตัวอย่างทั้งสิ้น 60 คน แบ่งเป็นกลุ่มทดลองและกลุ่มควบคุมกลุ่มละ 30 คน ในผู้ป่วยหลังผ่าตัดข้อไหล่และข้อศอก จำนวน 60 คน แบ่งเป็นกลุ่มทดลองและกลุ่มควบคุมกลุ่มละ 30 คน โดยใช้การสุ่มแบบบล็อก กลุ่มทดลองได้รับนวัตกรรมการป้องกันข้อติดร่วมกับการดูแลมาตรฐาน ออกกำลังกายวันละ 6 ชุด เป็นเวลา 3 วัน ขณะที่กลุ่มควบคุมได้รับการดูแลมาตรฐานเพียงอย่างเดียว เก็บข้อมูลก่อนและหลังการทดลอง ประกอบด้วย พิสัยการเคลื่อนไหวของข้อ (ใช้ universal goniometer) และความสามารถในการดำเนินกิจวัตรประจำวัน (ใช้แบบประเมินที่ดัดแปลงจาก Barthel Index) วิเคราะห์ข้อมูลด้วยการทดสอบทีแบบคู่ การทดสอบทีแบบอิสระ และการทดสอบไคสแควร์
ผลการวิจัย กลุ่มทดลองและกลุ่มควบคุมมีอายุเฉลี่ย 52.8 ปี (SD 11.2) และ 51.4 ปี (SD 10.8) ตามลำดับ เป็นเพศชายร้อยละ 56.7 และร้อยละ 60.0 มีค่าดัชนีมวลกาย 24.6 กก./ม² (SD 3.4) และ 24.9 กก./ม² (SD 3.2) ตามลำดับ จำนวนวันหลังผ่าตัดเฉลี่ย 1.8 วัน (SD 0.6) และ 1.7 วัน (SD 0.7) กลุ่มทดลองและกลุ่มควบคุมมีลักษณะทั่วไปไม่แตกต่างกันอย่างมีนัยสำคัญทางสถิติ (p > .05) ลักษณะทั่วไปของทั้งสองกลุ่มไม่แตกต่างกันอย่างมีนัยสำคัญทางสถิติ (p > .05) หลังการทดลอง กลุ่มทดลองมีพิสัยการเคลื่อนไหวของข้อเพิ่มขึ้นเฉลี่ย 15.2 องศา (SD 8.4) สูงกว่ากลุ่มควบคุมที่เพิ่มขึ้นเฉลี่ย 4.8 องศา (SD 6.2) อย่างมีนัยสำคัญทางสถิติ (p < .001) โดยเฉพาะการงอข้อไหล่ที่เพิ่มขึ้นมากกว่า 20 องศา ด้านความสามารถในการดำเนินกิจวัตรประจำวัน กลุ่มทดลองมีคะแนนเพิ่มขึ้นเฉลี่ย 19.3 คะแนน สูงกว่ากลุ่มควบคุมที่เพิ่มขึ้นเฉลี่ย 6.6 คะแนน อย่างมีนัยสำคัญทางสถิติ (p < .001) โดยผู้ป่วยในกลุ่มทดลองร้อยละ 73.3 สามารถพึ่งพาตนเองได้ในระดับดี เทียบกับกลุ่มควบคุมร้อยละ 26.7 ผู้ป่วยในกลุ่มทดลองมีความพึงพอใจต่อการใช้นวัตกรรมในระดับสูง (คะแนนเฉลี่ย 4.39 จาก 5.00) ไม่มีเหตุการณ์ไม่พึงประสงค์ร้ายแรงเกิดขึ้น
ข้อเสนอแนะ นวัตกรรมการป้องกันข้อติดมีประสิทธิผลในการเพิ่มพิสัยการเคลื่อนไหวของข้อ และความสามารถในการดำเนินกิจวัตรประจำวันของผู้ป่วยหลังผ่าตัดข้อไหล่และข้อศอก เป็นเครื่องมือที่ใช้งานง่าย มีต้นทุนต่ำ และผู้ป่วยสามารถใช้ได้ด้วยตนเอง เหมาะสำหรับนำไปประยุกต์ใช้ในการฟื้นฟูสมรรถภาพผู้ป่วยศัลยกรรมกระดูก อย่างไรก็ตาม ควรมีการศึกษาติดตามระยะยาวเพื่อประเมินความยั่งยืนของผลลัพธ์
