แนวทางการลดอุณหภูมิในผู้ป่วยบาดเจ็บสมอง ภายใต้หลักฐานเชิงประจักษ

บทคัดย่อ

การบาดเจ็บสมองชนิดรุนแรงเป็นภาวะวิกฤตที่มีความเสี่ยงต่อการเกิดการบาดเจ็บสมองระยะทุติยภูมิจากภาวะไข้ การสังเคราะห์หลักฐานนี้มีวัตถุประสงค์เพื่อรวบรวมองค์ความรู้เกี่ยวกับการจัดการอุณหภูมิในผู้ป่วยบาดเจ็บสมองชนิดรุนแรง โดยสืบค้นงานวิจัยที่เผยแพร่ระหว่างปี ค.ศ. 2016 - 2025 จากฐานข้อมูล ThaiJo, Google Scholar, PubMed และ Cochrane Library ตามแนวทาง PICO และคัดเลือกงานวิจัยตามหลัก PRISMA ผลการสืบค้นพบงานวิจัย 198 เรื่อง และผ่านเกณฑ์คัดเลือก 6 เรื่อง
ผลการสังเคราะห์พบว่า การจัดการอุณหภูมิประกอบด้วยการลดอุณหภูมิด้วยวิธีทางกายภาพ การควบคุมอุณหภูมิแบบมุ่งเป้าหมาย
และการรักษาด้วยภาวะอุณหภูมิต่ำ โดยการใช้อุปกรณ์ให้ความเย็นและการลดอุณหภูมิเฉพาะที่บริเวณศีรษะและลำคอมีประสิทธิภาพ
ในการลดอุณหภูมิร่างกายและความดันในกะโหลกศีรษะมากกว่าวิธีทั่วไป ขณะที่การรักษาด้วยภาวะอุณหภูมิต่ำแม้ช่วยลดความดันในกะโหลกศีรษะ แต่ยังไม่มีหลักฐานชัดเจนว่าสามารถลดอัตราการเสียชีวิตหรือเพิ่มผลลัพธ์ทางระบบประสาทระยะยาวได้
สรุปได้ว่า การควบคุมอุณหภูมิ โดยเฉพาะการลดอุณหภูมิเฉพาะที่และการรักษาระดับอุณหภูมิให้อยู่ในช่วง 36 - 37.5°C เป็นแนวทางสำคัญในการป้องกันการบาดเจ็บสมองระยะทุติยภูมิและสนับสนุนการดูแลผู้ป่วยบาดเจ็บสมองชนิดรุนแรงอย่างมีประสิทธิภาพ
