Effects of Case Management Program on Blood Sugar Control of Diabetic Patients in Nakhonchaisi Hospital

Abstract

Objective : To determine an ability of case management program on blood sugar control in type 2 diabetic patients.

Method : A Quasi-experimental method. The study subjects consisted of 30 type 2 diabetic patients. Fasting plasma glucose level was determined every trimester. Data was analyzed by descriptive statistics and repeated-measures one-way ANOVA.

Result : Thirty type 2 diabetic patients were recruited. The average age of patients was 52 years (mean 51.9). Diabetic time was ranging from 2 to 20 years (mean 7.2). The mean fasting plasma glucose level was 244.6 ± 41.5 mg/dL. Following the treatment with case management program, the mean fasting plasma glucose level was 166.6 ± 45.2, 164.8 ± 37.0, 153.2 ± 43.6 and 150.0 ± 32.7 mg/dL at first, second, third and forth trimester, respectively. And the mean fasting plasma glucose level was decreased 78.3, 79.7, 91.3 and 94.6 mg/dL at first, second, third and forth trimester, respectively. When compared the mean fasting plasma glucose level between, prior to and after intervention, it was statistical significance (p - value =.000).

Conclusion : The average fasting plasma glucose level after the intervention was decreased significance from prior to the intervention (p - value = .000). This study showed that case management program had ability to improve and long term control type 2 diabetic patients.
