Abstract

            This retrospective cohort study aimed to examine the survival rate of patients with end-stage renal disease (ESRD) receiving treatment at Muangsamsip Hospital, Ubon Ratchathani Province, from 2018 to 2025, and to compare survival rates among renal replacement therapy modalities. Medical records of 546 patients diagnosed with ESRD between 1 January 2018 and 31 December 2022 were reviewed, and all patients were followed up for vital status until 31 December 2025. Survival was analyzed using the Kaplan–Meier method, reporting the survival function, median survival time, and cumulative survival rates with 95% confidence intervals (CI). Survival rates between groups were compared using the log-rank test, with statistical significance set at 0.05.

Among the 546 ESRD patients (mean age 69.26 years, S.D. = 13.16) followed for a total of 1,416.98 person-years, 388 died, yielding a mortality rate of 27.38 per 100 person-years. The median survival time was 2.44 years (95% CI: 2.11–2.74). Overall survival rates at 1, 3, 5, and 7 years were 71.79% (95% CI: 67.82–75.37), 44.14% (95% CI: 39.89–48.29), 29.14% (95% CI: 24.93–33.46), and 11.82% (95% CI: 6.64–18.62), respectively. When survival was compared across treatment modalities, significant differences were observed (log-rank test, p < 0.001): median survival was 5.76 years (95% CI: 4.55–6.81) among patients receiving hemodialysis, 3.60 years (95% CI: 2.60–5.59) among those receiving peritoneal dialysis, and 1.45 years (95% CI: 1.15–1.79) among those who did not receive dialysis. In this study, ESRD patients receiving hemodialysis had the longest median survival, followed by those receiving peritoneal dialysis and those without dialysis, respectively. These modality-specific survival estimates can inform prognostication, treatment planning, and the care and follow-up of ESRD patients. Future studies should investigate the factors contributing to mortality in order to prolong patient survival.
