โรคเบาหวานชนิดที่ 2 ในการแพทย์แผนไทย: การสำรวจภาคตัดขวางของการปฏิบัติทางคลินิกและความท้าทายเชิงระบบ ในเขตสุขภาพที่ 12

บทคัดย่อ
การศึกษาภาคตัดขวางนี้มุ่งสำรวจแนวทางการวินิจฉัย การรักษา ความเชื่อมั่นของผู้ปฏิบัติงาน และความท้าทายเชิงระบบที่เกี่ยวกับการจัดการโรคเบาหวานชนิดที่ 2 ตามแนวคิดการแพทย์แผนไทย ในโรงพยาบาลส่งเสริมสุขภาพตำบล เขตสุขภาพที่ 12 กลุ่มตัวอย่างประกอบด้วยแพทย์แผนไทยจำนวน 117 คน เก็บรวบรวมข้อมูลด้วยแบบสอบถามออนไลน์แบบมีโครงสร้าง วิเคราะห์ผลด้วยสถิติเชิงพรรณนา การวิเคราะห์เชิงประเด็น และการทดสอบไคสแควร์ ผู้ตอบแบบสอบถามมีประสบการณ์วิชาชีพเฉลี่ย 6.3 ปี ให้การดูแลผู้ป่วยเบาหวานเฉลี่ยประมาณ 24.6 รายต่อเดือน ภายใต้แนวคิดของแพทย์แผนไทยโดยส่วนใหญ่ อธิบายว่าโรคเบาหวานเกิดจากความไม่สมดุลของธาตุไฟ โดยมีสาเหตุสำคัญจากพฤติกรรมการบริโภคอาหารที่ไม่เหมาะสม อาการทางคลินิกที่พบบ่อย ได้แก่ อาการชาปลายมือปลายเท้า แผลหายช้า เหงื่อออกมากผิดปกติ และกระหายน้ำบ่อย แพทย์แผนไทยมากกว่าครึ่งหนึ่งรายงานว่าใช้แนวทางการรักษาแบบบูรณาการ โดยผสมผสานการแพทย์แผนไทยกับยาลดระดับน้ำตาลในเลือดของแพทย์แผนปัจจุบัน ในด้านหัตถการทางการแพทย์แผนไทย พบว่าการประคบสมุนไพรและการอบไอน้ำสมุนไพรได้รับการใช้บ่อยกว่าการจ่ายยาสมุนไพร ตำรับยาเบญจกูล ตรีผลา และมะระขี้นก เป็นสมุนไพรที่ถูกสั่งจ่ายบ่อยที่สุด เกือบครึ่งหนึ่งของแพทย์แผนไทยรายงานว่ามีความเชื่อมั่นในระดับสูงถึงสูงมากในการให้บริการดูแลผู้ป่วยเบาหวานตามแนวทางการแพทย์แผนไทย นอกจากนั้นผู้ปฏิบัติงานได้สะท้อนถึงอุปสรรคเชิงโครงสร้างที่สำคัญ ได้แก่ การขาดแคลนยาสมุนไพรมาตรฐาน และความไม่เพียงพอของอุปกรณ์ที่เกี่ยวข้องกับการแพทย์แผนไทย ผลการศึกษานี้ชี้ให้เห็นถึงความจำเป็นในการพัฒนาแนวทางเวชปฏิบัติที่มีความเป็นมาตรฐานแต่ยืดหยุ่น สามารถบูรณาการการแพทย์แผนไทยร่วมกับการแพทย์แผนปัจจุบัน ควบคู่กับการสนับสนุนเชิงนโยบายที่เข้มแข็ง เพื่อให้เกิดการดูแลผู้ป่วยเบาหวานอย่างยั่งยืนและเป็นองค์รวมในระดับบริการปฐมภูมิ
