          การดูแลผู้ป่วยจิตเวชในชุมชนส่วนใหญ่เป็นหน้าที่ของญาติหรือผู้ดูแลขาดการมีส่วนร่วมของชุมชนในการดูแลผู้ป่วย ผู้วิจัยจึงทำการวิจัยเชิงปฏิบัติการ เพื่อพัฒนารูปแบบการดูแลผู้ป่วยจิตเวชในชุมชน ในเขตโรงพยาบาลส่งเสริมสุขภาพตำบลบ้านคอนสวรรค์ ตำบลไผ่ อำเภอรัตนบุรี จังหวัดสุรินทร์ กลุ่มตัวอย่าง คือผู้ที่มีส่วนเกี่ยวข้องการดูแลผู้ป่วยจิตเวชในชุมชนคัดเลือกแบบเจาะจงจำนวน 35 คน กลุ่มตัวอย่างเข้าร่วมกิจกรรมการวางแผนแบบมีส่วนร่วมโดยการสนทนากลุ่ม และดำเนินงานตามกระบวนการวิจัยเชิงปฏิบัติการ (PAOR) ระหว่างเดือนมีนาคม ถึงเดือนสิงหาคม 2558 เก็บข้อมูลโดยใช้แบบประเมินสมรรถภาพผู้ป่วยจิตเวชแบบสอบถามและแบบทดสอบความรู้ วิเคราะห์ข้อมูลโดยใช้ความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน เปรียบเทียบผลต่างค่าเฉลี่ยก่อนและหลังการพัฒนาโดยใช้ Paired sample t-test, Stuart and Maxwell Test กำหนดระดับนัยสำคัญทางสถิติที่ 0.05 และการวิเคราะห์เชิงเนื้อหา  ผลการวิจัยพบว่ารูปแบบการดูแลผู้ป่วยจิตเวชในชุมชนโดยการมีส่วนร่วมมี 4 ด้านคือ ด้านประชาชน ด้านภาคีเครือข่าย ด้านกระบวนการ และด้านการพัฒนาและเรียนรู้ การประเมินผลหลังการพัฒนารูปแบบ พบว่ากลุ่มตัวอย่างมีคะแนนเฉลี่ยการปฏิบัติในการดูแลผู้ป่วยจิตเวชอยู่ในระดับมาก ( = 2.65 ,SD. = 0.42) และความรู้อยู่ในระดับมาก ( = 16.40 ,SD. = 1.61) ซึ่งโดยรวมเพิ่มขึ้นจากก่อนการพัฒนาอย่างมีนัยสำคัญทางสถิติ (p<0.05) และผลการประเมินสมรรถภาพผู้ป่วยจิตเวชหลังการพัฒนาผู้ป่วยจิตเวชมีคะแนนการประเมินสมรรถภาพเพิ่มขึ้นอย่างไม่มีนัยสำคัญทางสถิติ (p-value=0.368) ปัจจัยความสำเร็จ เกิดจากการมีส่วนร่วมของชุมชนในการออกมาตรการ และใช้มาตรการนั้นภายในชุมชน และยังมีการดำเนินกิจกรรมเครือข่ายดูแลผู้ป่วยจิตเวชร่วมกันในชุมชนอย่างจริงจัง

The Development of Care Model for Psychiatric Patients in the community

Abstract

          Psychiatric care for patients in the community was mainly of relatives or caregivers, but also a lack of community involvement in patient care. This research aimed to develop a psychiatric patient’s care model in the community, in the catchment area of Baankonsawan Tambon Health Promoting Hospital, Tambon Phai, Rattanaburi district, Surin province. Thirty-five samples were purposive from the group of people who were involved in taking care of psychiatric patients in the community. Four steps of action research (planning, action, observation and reflection; PAOR), community participation and focus groups were used in the process of research methodology. All of the samples participated in the planning activities and joined in four processes of action research. This study was conducted from March to August 2015. The data were collected using the performance of patients with a psychiatric assessment form, questionnaires and knowledge assessment. The qualitative data was analyzed by a content analysis method. The quantitative data was analyzed by descriptive statistics. Paired sample t-test and Stuart Maxwell test were used to compare the results before and after the developed model, and the level of statistical significance is 0.05. The research found that the psychiatric patients’ care model in the community consisted of 4 parts; 1) people in the community, 2) the parties to the network, 3) the processing, and 4) the developing and learning. After evaluation, the development patterns, it found that the mean difference score of samples practical patient care psychiatry and knowledge were in good levels, which the overall increase significantly from before the development (p<0.05). Psychiatric patients’ performance evaluation scores after the development of psychiatric patients’ model increased but not statistically significantly (p-value = 0.368). The key success factors of the care model development for psychiatric patients consisted of the results from the participation of the community in the design and implementation of action within the community. There are also conducting psychiatric patient care network sharing in the community very seriously.
