Correlation between helicobacter pylori gastritis and gastric mucosal pattern by narrow band imaging endoscopy in Police general hospital

Abstract

Methods: Three hundred seventy seven patients undergoing Esophagogastroduodenoscopy (EGD)were prospectively enrolled. During the EGD, gastric body and antrum mucosal were Identified and examined using NBI prior to the before routine use rapid urease test for HP gastritis. Gastric mucosa patterns were divined into the following categories (Tahara et al): normal-small, round pits with regular subepithelial capillary networks; type 1-slightly enlarged, round pits with unclear or irregular subepithelial capillary networks; type 2-obviously enlarged, oval or prolonged pits with increased density of irregular vessels; and type 3-well-demarcated oval or tubulovillous pits with clearly visible coiled or wavy vessels.

Results:The study included 197 (52%) male and 180 (48%) female. The mean age and BMI of the patients were 59.48 and 25.45 respectively.Gastric mucosal pattern normal 294 (77.9%), abnormal type I 30 (7.9%), abnormal type II 38 (10%) abnormal type III 15 (3.9%). The sensitivity, specificity, positive predictive value and negative predictive value of abnormal NBI gastric mucosa pattern for determining H. pylori infection were 87.6% (95% CI 77-94) , 94.3% (95% CI 91-96), 79% (95% CI 68-87) and 96.9% (95% CI 94-98), respectively.

Conclusions:The use of NBI allows clear visualization of mucosal and capillary patterns. It provides great benefit for patients as it less invasive, and it is a cost effective alternative to rapid urease test for the prediction of H. pylori.
