Prevalence and Mortality of Pediatric Severe Sepsis and Septic Shock

Abstract

Objective: To investigate prevalence, clinical outcomes, mortality rate and factors associated with mortality in pediatric severe sepsis and septic shock.

Method: A retrospective cross-sectional descriptive study was conducted on patients aged 1 month to 15 years diagnosed severe sepsis and septic shock in the Pediatric Intensive Care Unit (PICU) of Mahasarakham hospital between 1 January 2018 to 31 December 2020. The data were collected and analyzed by descriptive statistics. Multiple logistic regression was used to identify factors associated with mortality.

Result: Of 592 pediatric patients admitted to the PICU, 52 patients were severe sepsis and septic shock. Prevalence of pediatric severe sepsis and septic shock was 8.8%. 61.5% had comorbid diseases. Almost patients were community-acquired infection (82.7%). The most common source of infection was the respiratory system (57.7%).  Only 25% had evidences of infection, most common organism wasE.coli (7.7%). 92.3% of patients had ≥ 2organs dysfunction. 76.9% of the patients received antibiotics within 60 minutes after diagnosis. Goal-directed therapywithin 6 hours and target urine output were achieved in 57.7% and 90.4%, respectively.The mortality among pediatric severe sepsis and septic shock was 28.8%. Factor significantly decreased mortality was achieved goal-directed therapy within 6 hours (Adjusted OR 0.05, 95%CI 0.00-0.76, p-value 0.032).

Conclusion: Prevalence of pediatric severe sepsis and septic shock was 8.8% and had high mortality to 28.8%. Achieved goal-directed therapy within 6 hours might be decreased the mortality.
