The The Practice of Nurses Practitioners In the context of Primary Health Care Thailand

Abstract

This focus ethnographic research aimed at studying the practice of nurse practitioners (NPs) under the primary care context in Thailand. A study setting were 43 health promoting hospital. Key informants were 43 nurse practitioners recruited by using selective criteria. A total of 24 general informants were the director of health promoting hospital, community nurse practitioners, registered nurses, a physiatrist, a public health technical officer, dental assistant, a physical therapist, a public health executive, a data entry operator, village health volunteers, a chief executive subdistrict administrative organization, the committee of sub-district health fund, the president and members of nurse practitioner club, and patients (or clients). Data were collected by using field mote taking, document analysis, in-depth interview, focus group, participant observation and non -participant observation. Data analysis were done with content analysis and thematic analysis.

Results indicated that the majority of NPs practiced under their clarity roles, regulation and functions of the primary care unit, responsible for promotive, curative, rehabilitative, and supportive health services for health needs of the community. They managed work by delegate work team activities in accordance with the professional scope and prioritized work team with the health data system given,  home visits for continuing care, managing flexible times to provide health services for the clients’ and community needs, examined health issues with the network of community leaders. The NPs created new health care services by conducting innovative projects for their communities and continuing knowledge development. Suggestions from these findings were that the policy development for establishing practice guidelines of primary health care services relevant to health problems and clients’ needs in every aspect by using a holistic approach under the work culture of those primary care units.
