Development Practice Guideline of Hypertension Patients for Chronic Kidney Disease Prevention in a Community in Nongdaeng Subdistrict, Si Chomphu District, Khon Kaen Province

Abstract

This collaborative action research aims to analyze the situations and development practice guideline of hypertension patients for chronic kidney disease prevention in a community in Nongdaeng Subdistrict, Si Chomphu District, Khon Kaen Province using the conceptual framework of the Expanded Chronic Care Model. Key informants, totally 48 persons, were purposively selected including hypertensive patients with uncomplicated renal conditions, caregivers and stakeholders. The data were collected through an interview, focus group discussion and observation. The quantitative data were analyzed using descriptive statistics. While content analysis was used for the qualitative data. This research was performed during August 2020 - April 2021. The results of the situational study revealed that most of the patients had a BMI considered to be chubby-obese, inappropriate health behaviors as well as regularly consumed high-sodium foods. The specific guideline was not found in the community health care. Also, the individuals in the community were not found participative in the patient care. Later, the participating stakeholders involved in a brainstorming session to create the development practice guideline of hypertensive patients for chronic kidney disease prevention in a community. The evaluation results showed that obtaining clear clinical services and follow-up home visits, up-to-date database connecting a community and family and commnity-based partcipation in the care of hypertensive patients could improve self-care and management in normal blood pressure control as seen in 3 (17.65%) kidney disease patients with increased glomerular filtration and 14 (82.85%) kidney disease patients with reduced glomerular filtration.
