HEALTH SERVICE SYSTEM DEVELOPEMENT FOR PATIENTS WITH UNCONTROLLED TYPE 2 DIABETES MELLITUS AT NAMNAO HOSPITAL, NAMNAO DISTRICT, PHETCHABUN PROVINCE

Abstract

This research aims to analyze the situation and develop a health service system for patients with uncontrolled type 2 diabetes mellitus. The concept of caring with chronic diseases patients (Expanded chronic care model : ECCM). Consisted of 8 components, this study focused on 3 components including (1) self-management (2) delivery system design and (3) Information system. Implementation period from March 2020 to April 2021. There are 4 steps of development process following the action research process. The results of the situation study revealed that diabetic patients and their caregivers did not have sufficient knowledge in self-management and manage. A large number of patients in service system and a limited number of health care providers. Database system clinics cannot be analyze. In development stage, the stakeholders gathered together for a brainstorming session. In the context of the urgency of Nam Nao Hospital's problems. Implementation plan including:1) Diabetes Self-Management Support System 2) Service system in the diabetes clinic of Nam Nao Hospital. 3) Nam Nao Hospital information system. The evaluation found that diabetes patients was better behavior modification. As a result, the level of glucose in the blood (HbA1C) was reduced by 14 people or 56.00 percent. A health service system for diabetic patients was developed. The database is analyzed and forward to allow patient for control blood sugar and prevent complications.
