THE DEVELOPMENT OF CARE SYSTEM FOR STROKE RISK GROUP AMONG HYAPERTENSION PATIENTS IN THE SERVICE AREA OF THAKHUNTHO HOSPITAL, THAKHUNTHO DISTRIC, KALASIN PROVINCE

Abstract

The 1^st cycle of action research aimed to study the development of care system for stroke risk group among hypertension patients in the service area of Thakhuntho hospital. The samples were divided 2 groups; stroke risk group with hypertension patients at 128 samples and stakeholders 30 samples. The instruments were used the quantitative data with questionnaire and qualitative data with in-depth interview, which implementation 8 weeks. The statistical was used descriptive statistic and content analysis.

The results showed; (1) The development of care system for stroke risk group among hypertension patients was born of situation and risk behavior among them, which was promoted health literacy, and supported and dissolved problem and barrier to caring stroke risk group from network partners to surveillance the risk of stoke among hypertension patients. (2) The implementation
the development of care system for stroke risk group among hypertension patients to try out and outcome of development of care system for stroke risk group among hypertension patients was found the workshop of stakeholders, the meeting for improve health literacy in community, the system to follow-up the risk group of stroke, the integration with multidisciplinary job in hospital and community, and development the potential between hospital,  municipality, and all of community sector for prevention stroke disease among hypertension patients in the service area.
